ConcertoHealth® Expands Presence in Detroit with New Clinical Administration Office in Southfield

DETROIT, Jan. 25, 2018 /PRNewswire/ — ConcertoHealth, the leading provider of specialized primary care and supporting clinical services for vulnerable, frail and elderly patients, announced today it is expanding in Michigan with the opening of a new administrative office in Southfield, Detroit. The office will serve as a clinical hub for the company’s innovative primary care support services model in the Detroit metro region.

ConcertoHealth supports health plans and their primary care provider (PCP) networks in caring for their most vulnerable patients. The ConcertoHealth Primary Care Support Platform combines its proprietary population health analytics platform, Patient^3D, with on-the-ground care teams including social workers, care managers, pharmacists, field-based clinicians, and dietitians. This care model integrates clinical care with social services to offer high-touch, individualized primary and preventive care for these patients, regardless of the care setting, be it at home, a doctor’s office, the hospital or post-acute setting.

In addition, ConcertoHealth operates a clinical hub in Kalamazoo, Mich., and four ConcertoHealth Care Centers in the Detroit metro area. The Care Centers offer services that go beyond the scope of a traditional doctor’s office to include 24/7 care coordination and support, and access to physicians, nurses, field-based clinicians, care coordinators, social workers, and pharmacists.

Since October 2017, ConcertoHealth has expanded its Detroit clinical hub team by 33 percent to 80 full-time employees. The new Southfield hub will house these team members, including care managers, and health plan and PCP support services staff. The team will deliver a patient-centric approach, central to the ConcertoHealth care model, to provide comprehensive telephonic and field-based care, and will also offer network support to health plan partners’ PCPs.
